The link below will get you to the forms you will need fill out for your camper’s attendance at YMCA Camp Santa Maria. The forms are in a PDF format that allows you to enter information into the form. And you will still have to sign several pages as well. If you have an electronic signature you can use that. You can also print them, sign, then scan them in and email them to us at santamaria@ymcabv.org . You can also drop them off at either branch or fax them to the following number: 303-664-5456.

Important notice:
We have found that when you download these forms on a Mac, there are some issues. We are working to correct them. You can do a few things until we fix the situation: Call or email camp and we can fax or mail you a copy of the forms, or you can download them onto a non-Mac system. We apologize for the frustration this may be causing.
On the forms, the second page of Form A, The Parent Contact information and releases shows as a blank page.
And in the Parent Packet, on page 5 there is a black and gray line that covers the words “Encourage your child to talk about their anticipation of camp. What they are excited…”
And on page 8, Health and Wellness the following does not show up:
“A physical exam is required for your child to attend camp.”
“The exam must be current within 24 months of months of the last day your child will attend camp.”
“The form must be filled out and signed by a licensed physician.”
