Registration for 2018 Summer Camp

Accredited and Licensed Camp Programs

To ensure the highest quality and standards, all of our camp programs are licensed by the State of Colorado. Due to licensing requirements, completion of all summer camp forms is a condition of participation in summer camp programs. All summer camp registration forms are available below. Forms are available in writable PDF format (latest version of Acrobat Reader recommended).

If you are registering for both day camp and Camp Santa Maria, or multiple camp sessions, you only need to fill out the General Information, Health History/Medical Release Forms and submit Immunization Records once. There are two additional supplemental health forms for Camp Santa Maria. You will also need to either have a physician complete Supplemental Health Page 4 or submit a physician physical that has been completed within the last 12 month of your child attending camp. Use the Camp Options and Payment Information forms to choose your camp sessions or register online.

Attention Before/After School Program Participants: If you are a 2017/2018 School Age Program participant, we already have your required health forms and only need your Camp Options, Payment forms and Policy Agreement. For Camp Santa Maria, we need the two additional Supplemental Health Forms, a physician physical completed within the last year and the Letter to Counselor Form for Camp Santa Maria.

YMCA Members: If you are a member of another YMCA rather than the Boulder Valley YMCA, our system will not recognize your member number or log-information as each YMCA association is a separate business entity. Once you are registered you can provide us with your YMCA member number and we can verify it with your Y and reduce your balance accordingly.  

How to Register for Camp Santa Maria Summer 2018

You can register your child online, via email, mail or in-person in a few simple steps:

  • After registering online, you'll receive a confirmation email with links to the required General Information, Health History, Parent/Guardian Agreements, Letter To Counselor & Cabinmate Request as well as the Summer Camp Parent/Guardian Packet.

Please note: You do not need to select a weekend stayover unless you are registering for multiple one week, consecutive camp sessions.
If you are registering or submitting forms within two weeks before the session start date, you must submit completed forms to santamaria@ymcabv.org or fax to 303-664-5456 and bring a copy of your completed forms and registration receipt with you to camp check-in.

Confirmation Forms

You should receive these forms in the email containing your receipt after you have registered online for camp.  If not, please fill out the following form to provide our office with more information and confirm your registration.

Bicycle Inspection Form

(Trekkers, Base Camp, and Outdoor Adventure Camp) In order to ensure the safety of participants, we ask that all campers (providing their own bike) to please have this bicycle inspection form completed by a bicycle mechanic in order to ensure that bicycles are properly tuned and fit and are in good working order. Most bike shops have mechanics that will perform the inspection for free. Camp Santa Maria has bicycles available if you cannot, or do not wish to, bring your own bicycle to camp.

Add/Change/Cancel Form

All camp adds, changes and cancellations must be submitted using this form. Refunds or credits, minus 25% of total fees paid will be authorized when a cancellation form is submitted at least 30 days in advance of camp start date. No credits or refunds without a 30 day written notice. No refunds will be given if there is a balance owed for any Y program.

Camp changes must be received at least one month prior to the start date of the camp you are changing to and a $25 change fee per week will be assessed.

For cancellations: You will be assessed the appropriate refund based on the amount paid and date cancelled per the refund policy.

Payment Options

Camp fees may be paid in full upon registration or a payment plan may be selected to be paid in full by May 1, 2018. If a registration occurs after May 1, camp fees are due in full upon registration. Participant must be the active member at time of registration through the duration of the registered program to receive member rates (M) or non-member (NM) rates will apply.

Payment plan details:

$200 will be paid at time of registration.

March 1, 2018: 50 percent of the remaining balance will be drafted from the original payer's account.

May 1, 2018: Remaining balance will be drafted from the original payer's account.

Wait Listing

Several of our summer camp sessions fill fast, so register early. If there is a camp session that is already full you may opt to add your child to a wait list. You will be notified within two weeks of the camp start date if space becomes open for your child.
